  9. This multipurpose form is designed for the lender to list their concerns that they would like the appraiser to comment on and to correct any errors or discrepancies found in the appraisal; AND can also be used to provide additional sales for reconsideration to the appraiser.

    Errors, discrepancies or special concerns to be addressed:
  15. SALES for reconsideration: When providing sales to be used as comparables, complete the section below. One line for each sale not to exceed 5 in total. The best sales for reconsideration are those that sold more recent, closer or that were more similar to the subject property. Include the address of the sale, the date of the sale, and any other information about the sale that you feel is relevant for its use.
